Transitional Care

We offer transitional care for seniors who are not quite ready to return home after being discharged from the hospital or skilled nursing care. Transitional care provides a short stay (less than 30 days) and includes all the services needed for healing quickly and comfortably. Our goal is to reduce unnecessary readmissions to the hospital and deliver a positive patient experience.

Services & Accommodations Include:

• 24-hour staff	• Apartments furnished and decorated
• Licensed nursing	• Chef-prepared meals available in the dining room
• Physical, occupational and speech therapies as needed	• Scheduled transportation available
• Medication management	• Housekeeping and linen services
• Private apartments with personal alert systems in case of emergencies	• A social calendar with daily opportunities for learning, creativity, fitness and fun

Transitional Care  – 24-Hours $200

Short-term stays are subject to availability. One does not have to be a resident to take advantage of this opportunity. Accommodations are private pay. For admissions and more information, please contact us.
