We offer transitional care for seniors who are not quite ready to return home after being discharged from the hospital or skilled nursing care. Transitional care provides a short stay (a minimum of 30 days) and includes all the services needed for healing quickly and comfortably. Our goal is to reduce unnecessary readmissions to the hospital and deliver a positive patient experience.

Short-term stays are subject to availability. One does not have to be a resident to take advantage of this opportunity. Accommodations are private pay. Stay is a minimum of 30 days – with a review every 30 days, up to 90 maximum days.
